MUST HAVE AN ACTIVE ENROLLMENT in the 90-day In-Home Help OR General In-Home Help Services to schedule under "ENROLLED CLIENTS: EXTENDED SERVICES"
From the first contact with a live case manager, the client will receive up to (2) hours of one-on-one consultation within seven (7) business days of outreach and advocacy support services with the assigned case manager.
The All-Inclusive 90-day In-Home Help Package offers a combined 20% discount on the service(s) provided to clients at an affordable monthly cost. Clients are scheduled for "XX" hours monthly for In-Home Service(s) based on initial assessment, which can include one or more, and up to all services, and receive the following benefits: direct contact with the assigned case manager, one-on-one outreach/advocacy, and access to respite care services, including overnights at an affordable out-of-pocket

The 30-Day In-Home Service and Extended Service Package includes: Clients are scheduled for "XX" hours of scheduled household/housekeeping duties, meal prep, shopping, transportation, personal care, medication planning, respite care, and assistance with activities of daily living. CLIENT/CAREGIVER MUST SCHEDULE EXTENDED SERVICES. PAYMENT IS REQUIRED UPON SCHEDULING SERVICE(S).
SERVICES LISTED BELOW ARE PROVIDED TO PERSONS WHO HAVE HAD A CONSULT OR ASSESSMENT FOR "START OF SERVICES" (schedule above)
Up to two (2) hours of minor in-home routine or as-needed maintenance and repair(s).
Up to two (2) hours of routine lawn care, seasonal clean-ups including main walkway, and mailbox (if applicable).
Up to two (2) hours of safety home modification(s).
